AGING PROGRAM

Prospect, Inc. provides Homemaker Services, Personal Care Services, and Respite Services through a contract with the Tennessee Commission on Aging and Disability and the Area Agency on Aging and Disability.

 

This program provides services to help individuals remain in their own home and community for as long as possible. It allows the individual to choose services and to be involved in planning their care. The purpose of the program is:  

  • To provide a choice between services in the home and nursing home care.
  • To prevent or delay the need for nursing home care.
  • To provide needed services less expensively than in a nursing home.
  • To allow the individual more choice in making decisions about care.
  • To provide services in home while maintaining a family-based support system.
  • To provide quality care in the individuals home to meet their needs.

The following services are available, by Prospect through this program:  

  1. Homemaker Services – This service, if needed, will provide assistance in the individual’s home with routine household chores. Examples would include sweeping, mopping, dusting, making the bed, washing dishes, personal laundry, ironing, and meal preparation. It includes errands such as prescription refills and grocery shopping.
  2. Personal Care Services – This service will provide assistance in the individual’s home with bathing, dressing, feeding, and other activities of daily living including assistance with household cleaning and the maintenance of a safe home.
  3. Respite – This service is provided on a short-term basis when there is an absence or need for relief of a caregiver. For example, respite might be provided when the caregiver is ill or hospitalized or when the caregiver takes a vacation. Respite is limited to a maximum of nine (9) days per year.

 

All services must be ordered by the individual’s doctor.  To qualify for the program, one must: 

  • Live in Tennessee;
  • Be 21 years of age and older;
  • Be eligible for Medicaid as determined by the Department of Human Services;
  • Have a medical condition that meets the TennCare requirements for nursing home care;
  • Need services in the home that would not cost more than care in a nursing home;  
  • Have a caregiver who is in the home 24 hours a day; and
  • Have a home that is a safe place to receive services to protect the health and safety of the individual.
